Sign up your business for a listing

Please submit the information below to be included in Value Card Program's online listing and have your offer displayed on the Value Card Program Web site in your chosen Market and Category. Your information will be reviewed and uploaded live within two business days after your payment is received.

IMPORTANT: The information you fill out below is the information that will be shown on the offer. If you don't wish a phone number or fax number or any other information to be shown in your offer, please don't include it here. Note that Contact and Email are required and are for our information only. They will not appear in your offer. After you fill out this form, you will be redirected to our shopping cart page where you will pay for your listing. The listing fee is $200 per year. You will fill out your billing information there. YOUR LISTING WILL NOT APPEAR ON THE VALUE CARD PROGRAM WEB SITE UNTIL YOUR PAYMENT HAS BEEN PROCESSED. IF YOUR PAYMENT IS NOT RECEIVED WITHIN TWO BUSINESS DAYS OF PROVIDING YOUR LISTING INFORMATION, YOUR LISTING WILL BE REMOVED FROM THE DATABASE AND YOU WILL NEED TO RESUBMIT YOUR LISTING.

Logo URL:	Logo guidelines: jpg or gif files only, size generally no wider than 150 pixels and no taller than 75 pixels. Click Browse to find the logo file on your computer. The logo will be uploaded when you submit this form. It's generally best if your logo is on a white background. If you don't have a logo, a generic word treatment with your business name will be created and used. Please give your logo file a name that is descriptive of your business, for example magnolia-pizzashack.jpg.
